Patient Information and Office Policies

APPOINTMENTS: If you need a special time for your appointments you may need to book weeks in advance.  Appointments are booked on a first come, first serve basis.  All minor patients must be accompanied by a parent/guardian.   Although we try to make reminder calls about scheduled appointments as a courtesy, we do not guarantee this service and it is the client’s responsibility to remember their scheduled appointments, and for telehealth appointments, to save their provider’s telehealth link to use at time of the scheduled appointments. Telehealth links are provided for the first appointment and are always included on this website for client convenience.

All of our providers try to see clients at their appointed time, however, please be aware that emergencies may need some extra time and the providers run late on occasion.  If you are required to wait more than 60 minutes to see the Physician you may reschedule your appointment without incurring any charge.  If you leave prior to the 60 minute time limit, you may incur a charge for a missed appointment.  

PRESCRIPTIONS: Prescriptions are provided at the time of your appointment. Medications which require written prescriptions need to be handled at the time of your appointments. If you require mail order prescriptions, please request them from your physician at the time of your appointment. cLIENTS will be required to send in the written prescriptions to your mail order pharmacy.  We do not fax or phone in mail order prescriptions.  

EMERGENCIES: Psychiatric emergencies include patients who are acutely ill, having severe medication side effects, suicidal or whom may harm others.  Should you or a loved one be experiencing a psychiatric emergency, please call 988 or go to your nearest emergency room.

INCLEMENT WEATHER: In cases of in person appointments during severe weather, if it is necessary to make alternative arrangements for the appointments, we do try to contact clients and give notice.  Please call the office or email support@lchbehavioralhealth.org to check about office status before traveling to the office. FEES: For clients with insurances that we participate with, we accept contracted insurance rates, however given the high demand for services, initial appointments require a $75.00 fully refundable deposit prior to scheduling. This amount will be credited to your account unless the initial evaluation appointment is canceled or missed for any reason. Insurance companies require clients to pay their deductible and copay (portion not covered by insurance) at each visit until such time that the deductible has been met. Please speak with your insurance company directly for questions or concerns.

Initial assessment fee is $350.00, 45 min session fee is $285.00, 30 min session fee is $200.00. Psychiatric medication management 15-20 min session fee is $165.00. Fees for extensive phone calls, family consultations, consultations with other physicians and therapists are not covered by insurance and may be billed at the above rates based on time spent.

Your appointment time is reserved just for you. We understand that things happen, and you may need to reschedule your appointment, however we require 48 business hours’ notice. Unfortunately, missed appointments and/or sessions canceled within 48 business hours from the appointment time are not covered by insurance and will be charged $50.00 for the first occurrence. Subsequent missed or late canceled appointments are subject to full fee. Frequent cancellations will be reviewed for continued services. There is no charge for sessions that are missed or canceled due to the provider's absence.

Insurers also don’t consider providing letters and documentation as covered services. LCH Behavioral Health established an administrative charge for letters, records, and short (1 page) forms at $50.00 and for longer forms and disability applications at $150.00.  We retain medical records for 12 months after a lapse in treatment.  After 12 months, medical records are archived off premises.  There is an additional charge of $25 to have your medical record retrieved from archives. Any consultation for legal reasons will be billed at the rate of $300 per hour and are payable by the client at the time of service. These charges cannot be reimbursed by insurance. Court appearances, testimony, response to subpoena will be charged at the rate of $1000 per day and is payable by the client at least three business days in advance of such services to be provided.

    Unfortunately, we are unable to participate in Medical Assistance at this time.

    We require that you present all of your insurance information as well as a copy of your insurance card and furnish a proper referral if it is required by your insurance company.  The client will be held responsible for the total bill incurred if inaccurate or invalid insurance information is provided. Please remember, your insurance policy is a contract between you and your insurance company.   Our office does not guarantee the accuracy of your copayments or of the amount your insurance company will pay.   You remain responsible if your insurance company does not pay or if they pay incorrectly.  Please also be aware that some services may not be covered under your plan or may not be covered up to the amount charged.  These charges will be due immediately upon notification. Account balances must be paid within 30 days. Account discrepancies must be settled within 60 days.  Interest will be charged at the rate of 1.5% (18% annually) for any balance more than 30 days old.  There will be a charge of $25 for any returned checks.
